It was not that long ago, that the discovery of an MB2 canal in a maxillary first or second molar was considered an "occult" experience. Their existence slightly less than 100% of the time was subject to derision. And yet, when that particular root was surgerized, it was invariably evident that a second primary untreated/unfilled canal was an etiologic factor in the related pathology.

Four canals in a mandibular molar is customary, and yet, how often is a mesial root surgerized where an isthmus communication between the MB and ML canal is NOT detected? It is time to energize ourselves to the pursuit of the uncovery of the MM (middle mesial) canal. We have known of the exquisite complexity of the root canal system for over a hundred years?.it's time to recognize that science and clinical acumen must meld to achieve 100% predictability in pursuit of endodontic success
